INSTRUCTIONS .
This form is a required addendum for all applications made on Form TC106. It is also always required for
applications for condominium property on Form TC109 if the applicant’s claim that an individual condominium unit is overvalued stands
independent of the correctness of the entire condominium’s assessment as a whole.
If this addendum is not attached to the application form, it may be attached to a supplemental application Form TC150 and filed by
March 24. If there is a transfer among related parties or change in the applicant's relation to the property after the application is filed,
this addendum is the way to supply that information by attaching it to a supplemental affidavit (Form TC159) and submitting it at the
hearing. Use TC230 and TC159 to report a sale to a non-related party after the application is filed.
If you are required to attach this addendum, answer the applicable questions in Parts 1 and 2. Parts 3 through 9 do not apply to all
applicants. Complete parts that apply. Generally, condominium unit applicants should complete Parts 1, 2, 3, 7 and 8.
