printed on this form.

You will be advised of the results of these enquiries in approximately six to eight weeks.

You may apply for a Certi cate of Entitlement or request insurance cover before a service check is completed, but we
cannot issue a Notice of Eligibility or a Certi cate of Entitlement until it is nalised.
•
Please contact DSH before entering into a contract for your housing approval. It is important that you do this so that your
entitlement to a DSH loan can be established before you incur any costs.
