the owner of record, and that I am liable for paying the taxes on this

property. I am an Illinois resident and I have served in the United States

Armed Forces, the Illinois National Guard, or Reserve Forces, and have

received an honorable discharge. I have a service-connected disability of

(check only one):

____ at least 75% and am entitled to an annual exemption of $5,000 from

my equalized assessed valuation; or

____ at least 50%, but less than 75% and am entitled to an annual

exemption of $2,500 from my equalized assessed valuation.

In support of this application, I hereby supply a copy of the first two pages

of my Award Letter. (Applicable documentation must be attached to this

form).

Pursuant to 35ILCS 200/15-169, this exemption is limited to residential property with an equalized assessed value of less than

$250,000. This exemption will carry to the benefit of the veteran’s surviving spouse as long as the spouse holds the legal or

beneficial title to the homestead, permanently resides thereon, and does not remarry.
