AGREEMENT

_______________________________ (hereinafter “User Group”) and the State of Rhode

Island Department of Environmental Management, Division of Parks and Recreation

(hereinafter “Division”) the parties to this agreement, state that:

Whereas, User Group is an organization desiring to obtain a permit and/or pass

allowing its members to use State Facilities; and,

Whereas, some or all members of user group are or may be under the age of

eighteen (18) years; and,

Whereas, Division is concerned with the Health and Safety and right of quiet

enjoyment of the General Public, other users of the facility and the members of User

Group, and is future concerned with the proper use of the facility;

Now, therefore, in consideration of mutual covenants and agreements contained

herein. Division will issue such permit and/or pass provided that User Group at all times

Provides adequate adult supervision for the members of its group under the age of

eighteen(18) years to ensure their and proper use of and respect for the facility as well as

ensuring the safety and right to quiet enjoyment of other users and the Public in General.
