The Appraisal Board can reissue an appraiser pocket card only if the original card has been lost, damaged, destroyed, or never
received. If the name of the registration, license or certificate holder has been legally changed, by marriage or otherwise, please
submit the form, Request for Name Change, which can be found on our website.
To obtain a replacement pocket card, you must properly complete and file with the Appraisal Board this form with the required
$10.00, or $ .00 if being added to the National Registry. [Note: No fee is required if pocket card was never received (e.g., lost by
Post Office).]
