I had circumstances beyond my control that directly affected my academic progress. Attach letter and supporting
documents.
ATTACH REQUIRED DOCUMENTATION: A typed letter explaining your mitigating circumstances. Mitigating
circumstances are defined as unanticipated and unavoidable events or situations beyond a student’s control that prevent him/her
from successfully completing a course. Your explanation MUST:
1.
State how your circumstances directly affected your academic progress at UNCP.
2.
Explain how you have resolved your circumstances
3.
Include documentation to support the explanation.
ATTACH SIGNED FINANCIAL AID ACADEMIC IMPROVEMENT PLAN completed through the Academic Support
Center. Call (910) 521-6625 to make an appointment.
After completing additional credits at UNCP, I have now met all three of the SAP standards and I am requesting a
reevaluation of my academic progress.
By signing this form, I certify that I have read the appropriate policy and the information I have provided is accurate, true, and complete.
My appeal will not be considered, if any item is missing, including signature and/or a balance from a previous term is owed, (if
applicable). I understand that submission of this appeal does not guarantee reinstatement of academic or financial aid eligibility and that I
am responsible for all charges, both prior and current, regardless of whether a decision has been made on my appeal at the time payment
comes due and/or regardless of the outcome of my appeal. I also understand the decision of the Appeals Committee is final.
