**Activities located partially within a development subdistrict and either a management or protection
subdistrict will require review by both the MFS and the LUPC. Please fill out one application, make a
photocopy and send a copy to each agency.

Note:
The accompanying application is a LUPC form that is in the process of being revised to reflect
the changes outlined above. The MFS is using this LUPC form temporarily while updated forms are
being developed. Consequently, there may be places where references to the
LUPC’s Chapter 10 Land
Use Districts and Standards
will be replaced with references to the
Maine Forest Service Chapter 27
Rules. Chapter 27 is now in effect.
