I hereby elect a court hearing regarding the above-listed civil citation and enter my plea as follows:

I understand that the court may impose a civil penalty of up to $500 if I am found to have committed a traffic
violation other than unlawful speed in a school or construction zone. For unlawful speed in a school or construction
zone, the court may impose a civil penalty of up to $1,000. I further understand that the court may assess statutory
court costs and, if I request an evidentiary hearing but decide at that hearing to enter a guilty/no contest plea, the
court may assess additional court costs to cover expenses such as the cost of unnecessarily requiring the attendance
of witnesses.
