To Telecommunication Provider:
The filing of an annual report is required, by law, of every public utility holding a Certificate of
Public Convenience and Necessity (“CPC”) to operate in Nevada.
Pursuant to Nevada
Administrative Code (“NAC”) 704.7483, every telecommunication provider possessing a valid
Nevada CPC must submit an annual report to the Public Utilities Commission by May 15.
Enclosed is the list of information that must be included in your Annual Report for the 2007
calendar year as required by Nevada Revised Statutes (“NRS”) 704.68871 and NAC 704.7483.
Items 1 through 9, as shown in the following report, must be submitted even if the
telecommunication provider offered no service in Nevada.
If the telecommunication
provider cancelled its CPC during the calendar year, it still must provide an annual report.
Please note that NRS 703.380 allows for any public utility which violates any provision of NRS
Chapters 703 or 704 to be liable for a civil penalty of up to $1,000 per day for each violation.
Failure to file an annual report could also result in revocation of your CPC.
If the current address or contact information for your company has changed, you must notify the
Commission in writing of the change. This is also required for any future changes. Please note
that the Commission only maintains one address. It is the company’s responsibility to forward
correspondence to the correct party.
For the Commission’s convenience, please make sure that any annual report submitted is not
permanently bound. This will enable the Commission to provide copies of annual reports to the
public.
