Complete this form to request a bad debt credit for Nevada sales tax paid but for which you are unable to collect.
You may only request credit for bad debt if:
1) You are a retailer of tangible personal property, and
2) The debt is not attributable to a third-party transaction, and
3) You have filed a Federal return with the Internal Revenue Service claiming the bad debt deduction.
A request may only be granted to the extent Nevada sales taxes were paid to the Nevada Department of
Taxation.
