Instructions for Filing

APPLICATION TO REGISTER A LIMITED LIABILITY COMPANY NAME

(Form L-05)

Check the appropriate blank for an initial or renewal application.

Item 1

Enter the complete limited liability company name to be registered exactly as it appears

on the Certificate of Existence, or that name with any addition required by N.C.G.S.

§57C-7-06.

Item 2

Enter the name of the state or country of organization and the date of organization.

Item 3

Enter the type of business in which the limited liability company is engaged.

Item 4

See form.

Date and Execution

Enter the date the document was executed.

In the blanks provided enter:

•

The name of the limited liability company registering the name.

•

The signature of the representative of the limited liability company (may be

manager, or if managers have not been selected or of the limited liability

company does not have a manager other than a member, by any member).

•

The name and title of the above-signed representative.
