Specific Instructions For Form T-10 Worksheet
Producer’s Statement of Oil Production
Do Not Report Production From A Secondary or Tertiary Project On The T-10 If The Project Has Been
Certified For The Incremental Oil Exemption And The Project’s Five Or Ten Year Exempt Period Has Begun.
Please Contact The Office of State Tax Commissioner For The Appropriate Forms And Instructions To
Report Incremental And Nonincremental Oil Produced From The Project During The Exempt Period.
LINE INSTRUCTIONS
Producer’s Name & Federal I.D.: Print name and nine-digit Federal I.D. number on the top left portion of the worksheet and enter
the page number on the top right portion of the worksheet.
Month & Year: Enter numerals, e.g. 0 7 9 6 for July 1996.
SEPARATE LINE ENTRIES ON THE WORKSHEET ARE REQUIRED IN THE FOLLOWING SITUATIONS FOR OIL
PRODUCED AND SOLD FROM A SINGLE WELL:
a) Test oil recovered from a new well.
b) Condensate recovered from the wet gas stream (see the General Reporting Instructions #14 for procedures on reporting
condensate).
c) Oil produced from different reservoirs (i.e., pools).
d) Oil produced from a new well during the 15-month exempt period or a new horizontal well during the 24-month exempt
period but sold after that period has ended. (The 15 or 24 month period begins on the date of well completion and runs for
15 or 24 consecutive months.)
e) Oil produced from a qualified work-over project well during the 12-month exempt period but sold after that period has
ended. (The 12-month period begins on the first day of the third month after completion of the work-over and runs for 12
consecutive months.)

Oil produced from a well before the first day of exempt stripper well status but sold after the exempt period begins. (A
stripper well’s exempt period begins on the first day of the month following the 12-month qualifying period.)
g) Oil produced from a qualified horizontal reentry well during the 9-month exempt period but sold after that period has ended.
(The 9-month period begins on the date of well completion and runs for 9 consecutive months.)
h) Oil produced from a qualified two-year inactive well during the 10-year exempt period but sold after that period has ended.
(The 10-year period begins on the first day of the month in which the certification is received by the Commissioner.)
API #: The API number for an individual well, group or unit must be provided in order to process the lease detail information. To
report on an individual well basis, please enter the ten-digit API number assigned by the North Dakota Industrial Commission. The
first two digits of the API number designate the state code (i.e., #33 for North Dakota) and have been provided on the report for your
convenience. The state code is followed by the three-digit county code and five-digit well number. The five-digit well number is not
the same as the file number assigned by the Industrial Commission.
If reporting on a property basis, please enter the nine-digit API number for the group or unit assigned by the Commissioner. The API
number for the group or unit uses the same state and county code designations. The four-character property identification number for
the group or unit will consist of an alpha character followed by three digits.
T/C: If reporting test oil recovered from a new well, enter T. If reporting condensate recovered from the wet gas stream, enter C.
Otherwise, leave this space blank.
Pool Code: Enter the pool code assigned by the Commissioner. If reporting oil production from a well which produces from more
than one pool, enter the applicable code assigned to each pool. A list of assigned pool codes can be found in the Special Instructions
section of this booklet. Please contact the Commissioner if you are unable to determine a required pool code.
