Documentation Required With License
Application
Licenses will only be issued when the following documentation is received.
All applications:
Completed ASN license application form.
Copy of current membership card in a club, which is affiliated to Jamaica Millennium Motoring Club.
A completed ASN Medical form as follows:
o
All first time License applicants must submit a medical
o
International License applicants must have a medical every year
i.
a Stress ECG is required every two years if applicant is 45 or older
o
National License applicants must have a medical:
i.
every 5 years between ages 9 & 35
ii.
every 2 years between ages 36 & 59
iii.
every year over 60 years of age
iv.
Medical History Self-Declaration Form to be completed and submitted on the years when a
Physical Examination is not required (see above)
NOTE: Applicants should keep a copy of their completed medical form
Copy of most recent or current competition license.
Proof of recent competition experience (e.g. Copy of drivers log card, event results or series results).
Appropriate fee, made payable to Jamaica Millennium Motoring Club (JMMC) by cheque or money order.
ALL License applicants must also attach the following:
A high resolution head shot photo in JPEG format, 2 inches wide (50 mm), to be
emailed to
New Race applicants must also attach the following:
Copy of racing school certificate with Chief Instructor's endorsement (for example IP45 training through
JAMSPEED/Jamaica Race Drivers Club. - Contact the JRDC office for further details)
Proof of age (e.g. Copy of a Birth Certificate, Driver’s License, or Passport)
New Kart applicants must also attach the following:
Proof of age (e.g. Copy of a Birth Certificate, Drivers License, or Passport)
Upgrade applications in the same year must also attach the following:
Current race competition license (not a copy)
NOTE: JMMC must receive your current license before the upgrade license can be issued. If your
application for an upgrade is not approved your current license will be returned to you.
Applicants under the legal age of consent in Jamaica must also attach the following:
Signed “Consent and Release by Parent or Guardian”
