Optician Registration Application Form

Dear Applicant,
Thank you for your interest in registration with the College of Opticians of Ontario (the COO).
The following are the registration requirements for a certificate of registration (certificate) as a
Registered Optician with the COO:
1.
Application Form: Completed ‘Optician Application Form’ (see below);
2.
Combined registration and application fee: The combined registration and application fee is pro–rated, based on
the time of year your application is processed. *All fees include HST and must be made payable to the COO by either
Visa or MasterCard. Payment by personal cheque, certified cheque, money order, or cash is not accepted and will be
returned; below;
3.
One colour passport-sized photograph: Please

4.
Completed ‘Verification of Dispensing Experience Form’ (see form below) or Proof of Completion of
Required Dispensing Experience from Teaching Institution: *Only applicable to “Accredited” and
“Non-
Accredited” education applicants;
5.
Proof of graduation: If not already submitted, “Accredited” and “Non-Accredited” education applicants must provide
proof of successful completion and graduation from an accredited education program or an unaccredited program
(Canadian or International) approved by the Registration Committee of the COO.
6.
Proof of successful completion of the ‘National Optical Sciences Examinations’ (Contact Lens and
Eyeglasses): If not already submitted, “Accredited” and “Non-Accredited” education applicants must provide proof of
successful completion of the examinations offered by the National Association of Canadian Optician Regulators
(NACOR).
7.
Documentation to verify your legal name: Applicants who have never held a certificate of registration with
the COO must submit a notarized photocopy of their current passport, or driver’s license, or birth certificate.
*
If
you are submitting your birth certificate and the name that appears on it is not your current legal name you are
required to submit secondary proof of your legal name (E.g. Current passport or drivers license) ;
8.
Documentation to verify your citizenship status in Canada: Applicants who have never held a certificate of
registration with the COO must submit a notarized photocopy of either, their Canadian passport, or Canadian birth
certificate, or permanent resident card, or record of landing (Pre 2001), or confirmation of permanent residence
(Post 2001), or current work permit, or current student visa.
*Submission of a notarized photocopy of a current Canadian passport or Canadian birth certificate is sufficient
documentation to verify both your legal name and Canadian citizenship.
9.
Certificate of Being Insured under a Professional Liability Insurance Policy Form (‘COBI’), OR ‘Undertaking
Form: See Below.

10.
Letter of Standing: If not already submitted,
“Nationally
Registered” applicants must provide a
‘Letter of
Standing’
from all provinces they are registered in as an optician;
11.
Proof of education completed inside or outside of opticianry: If not already submitted and if applicable;
12.
Details of ‘Conduct Declaration’: If you answer ‘YES’ to any of the conduct questions, please provide full details
including any relevant documentation. For further guidance, please also click here to refer to the Registration Policy
dealing with 'Convictions, Findings and Ongoing Proceedings';
13.
Details if not in good standing in ‘Concurrent Registration Location(s)’: Provide full details as requested on the
application form if you are currently registered/license to practice opticianry or, another regulated profession other than
opticiancry, in a province other than Ontario or, in another country other than Canada.
Processing Timeline and Accommodations of Special Needs:
The processing of the registration application

Final Approval:
You will be notified of any outstanding requirements with your registration application by email.
Please notify the College immediately of any change in your email address to ensure receipt of our correspondence. All
applicants will be notified by mail of the outcome of their application. Upon final approval of your application you will
receive a package by mail containing the following:
1. A letter confirming approval of your application;
2. Your photographic identification badge;
3. Your confirmation of payment receipt.
