The second quarter 2006 brand specific report is now online for your convenience. This fill-in
version of this form is on the Department's website
which will be in acrobat
reader. You open the form and then you can type in the information on your computer. The
pages will total for you automatically. Print one copy to mail to the department (keep a copy for
yourself). There is not a way available yet to electronically file this form. Please send only the
original report, no extra copies are requested at this time.
If you use an Excel file or computer-generated report, you may still send those attached to a copy
of the Brand Specific Report. You must still fill in the distributor information at the top of the
page. Also, make sure that your monthly reports are combined for a quarterly total. If there is
not ONE quarterly total for each brand, the report will be considered as incomplete and returned
to the distributor for completion.
For TOBACCO ONLY DISTRIBUTORS, you must complete this form each quarter, if you are
purchasing roll your own (RYO) products. We need to know if you are purchasing any RYO
products directly from the manufacturers, which is then sold in Iowa. If you are not buying RYO
product, just fill in the permit number and the distributor information at the top of the page, check
the NONE box, sign and date the form. ALL IOWA Cigarette or Tobacco Distributors, whether
there is Iowa purchases or sales must file this report. Failure to timely file this report each
quarter will subject you to a penalty of $200.00 or greater.
All information pertaining to the Directory of Approved Brands and the Minimum Price List is
only sent out by e-mail to all subscribers of the cigarette and tobacco eList. Please contact
Dawn Johnson if you are not currently on the eList or for some reason have not received an
e-mail from the department within the last month. All cigarette and tobacco tax forms need to be
retained for three years, unless it pertains to the Brand Specific Reports, which have a five-year
retention requirement.
