INSTRUCTIONS FOR APPLICATION FOR ALASKA CERTIFICATE OF AUTHORIZATION

FOR CORPORATE, LIMITED LIABILITY COMPANY (LLC) OR LIMITED LIABILITY PARTNERSHIP (LLP)

PRACTICE OF ARCHITECTURE, ENGINEERING,

LAND SURVEYING, OR LANDSCAPE ARCHITECTURE

1. Read Alaska Statutes and Regulations for Architecture, Engineering, Land Surveying, and Landscape Architecture, in

particular AS 08.48.241 CORPORATIONS, LLC, LLP, and 12 AAC 36.135. REVIEW OF APPLICATION FOR

CORPORATE, LLC, OR LLP AUTHORIZATION (see attachment)

2. Obtain certificate from Alaska Division of Banking, Securities, and Corporations. (See address below.)

3. Complete Form 08-610, APPLICATION FOR CERTIFICATION OF AUTHORIZATION FOR CORPORATE, LLC, OR

LLP PRACTICE OF ARCHITECTURE, ENGINEERING, LAND SURVEYING, OR LANDSCAPE ARCHITECTURE and

mail to address at the top of this page.

APPLICATION MUST BE TYPED AND MUST INCLUDE THE FOLLOWING:

1. Fee of $285, made payable to State of Alaska.

2. List of ALL business names under which the corporation, LLC, or LLP provides services.

3. Photocopy of certificate from Alaska Division of Banking, Securities and Corporations.

4. Certified notarized copy of amendment to bylaws, which includes provisions under AS 08.48.241(b)(1).

5. Certified notarized copy of resolution of corporation’s board of directors, LLC’s managing members or

manager, or LLP’s general partners designating person(s) holding certificates of registration to practice in

Alaska as being in responsible charge of each branch of corporate, LLC, or LLP practice, as specified

under AS 08.48.241 (a) (2) and (3). All persons designated in responsible charge must hold a current

Alaska registration. See sample resolutions attached.

6. Evidence the corporation, LLC, or LLP has the ability to provide architectural, engineering, land surveying,

or landscape architectural services by stating the experience of the corporation, LLC, or LLP during the

preceding five-year period, or any other information which reflects the corporation’s, LLC’s, or LLP’s ability

to provide service, such as company brochures, etc.

7. Statement of the professional record of each person designated in responsible charge for the

corporation’s, LLC’s, or LLP’s activities.
