All continuing education participants must complete this form for each book they read. This form must be completed and

submitted to the center before the appropriate CEU will be officially noted on their participant enrollment report. Each book

read is given 1 CEU no matter the length of the book.

NOTE: There is a $10 processing fee for recording this continuing education experience for each book read.

Each book reported must have its own form and processing fee.
