Distant Customer Tuxedo Measurement Form
For use by those for whom it is not possible to physically visit Top Shelf Menswear for tuxedo selection and measurements.
Instructions
1. Please have your measurements taken by a professional – most will perform the service for a nominal fee.
Recommended professionals: Tailor Shop, Mens’ Clothing Store, Tuxedo Rental Store.
We do not recommend that you measure yourself or use clothing size tags.
2. Complete the form as accurately as possible and mail or email to the above address, (At least 10 days before the event)
3. Orders must be accompanied by a $75 deposit. Balance due when tuxedo is picked up.

Rental Terms for Apparel & Accessories
1.
Please plan for an in-store final fitting prior to use to ensure the appropriate fit of the garment. Customers declining this fitting will
assume full responsibility, as we cannot be held responsible for measurements not performed in our store. We will make every effort to
deliver a quality, well-fitted garment, but customer acknowledges that this may require additional fees, such as rush shipping and/or
additional tailoring fees.
2.
A non-refundable deposit of $75 is required at time of reservation. Balance due upon receipt of tuxedo &/or accessories.
3.
Changes made by the customer within 7 days before date needed may require additional fees.
4.
Customer agrees to return this rental by the first business day following use, or pay a $25/day late fee.
5.
All rentals require a $6 damage waiver, which releases the customer from liability for minor wear and tear, but does not cover theft,
loss, malicious damage or damage that prevents future use of the garment. Lost or stolen items will be charged at replacement cost.
(Approximate replacement charges for individual items is available upon request.)
6.
Customer agrees that management & employees of Top Shelf Menswear are not responsible for personal items left in the apparel.
7.
Customer agrees that charges assessed for damage, loss or delayed return may be charged against the payment method indicated below.
