Send the entire form, a check payable to the County of Fairfax, a copy of rabies

certificate and proof of neutering, if applicable, to:

Dog licenses are available at:

The Department of Tax Administration, 12000 Government Center Parkway, Suite 223, Fairfax,

between 8:00 a.m. and 4:30 p.m. weekdays (8:00 a.m. to 6:30 p.m. Thursdays),

The Animal Shelter, located at 4500 West Ox Road, Fairfax, between 8:00 a.m. and 6:00 p.m. Tuesday

through Friday and Saturday between 8:00 a.m. and 5:00 p.m..
