Your signature is required below, and certifies:
• You have enclosed with this application the proof of liability insurance required in RCW 79A.60.480 (2)(c), and
that you will continuously maintain the liability insurance coverage for the time you are licensed. (The insurance
requirements are listed on the reverse.)
• All employees, subcontractors, and/or independent contractors hired, or used by your business as guides meet, or
will meet the requirements set by RCW 79A.60.430 (2) before conducting any passengers for hire on a section of
river designated as whitewater. (The guide requirements are listed on the reverse.) RCW 79A.60.480 (2)(b)
• That at no time during the past twenty-four (24) months immediately before this application you or any business for
which you were directly responsible has had a license, permit, or certificate to carry passengers for hire on a river
revoked by another state or by an agency of the United States government due to a conviction for a violation of
safety or insurance coverage requirements, as long as those requirements were not more strict than those within
Washington State. RCW 79A.60.480 (2)(e)(i)
• That at no time during the past twenty-four (24) months immediately before this application you or any business for
which you were directly responsible has been denied the right to apply for a license, permit, or certificate to carry
passengers for hire on a river by another state. RCW 79A.60.480 (2)(e)(i)
• That all persons conducting whitewater river trips under the authority of your whitewater river outfitter license will
have proof of the license and be able to show it to law enforcement officials at either the site where the vessels are
put into the river or the site where the vessels are taken out of the river. WAC 308-312-100.
