Pursuant to A.R.S. § 29-805

Articles of Amendment



Indicate the exact name of the limited liability company in Arizona, including the

appropriate corporate identifier (i.e. L.L.C.).



The Articles of Amendment must be executed by a manager if management of the

foreign limited liability company is vested in a manager or by a member if management is

reserved to the members.



If changing the name of the foreign limited liability company, you can check the

availability of the name and reserve it for up to 120 days on our web site at:

/divisions/corporations.

Copies



Attach a certified copy of the amendment, restated articles, merger or other document

that has been filed in the domicile state. The copy must be certified within sixty (60) days

of delivering to the Commission by the official having custody of corporate records in the

state, province or country in which the company is organized.

Fees



Attach check for the filing fee of $25.00. Expedited service is available for an additional

$35.00. Make check payable to the Arizona Corporation Commission.
