APPLICATION & INSTRUCTIONS FOR A
SPECIAL EVENT LICENSE
CODE-214
A circus, redo, carnival, fair, performance, singing, playing of musical or other instruments, dancing or amuse-
ment of any kind, or preaching temporary structure of any kind, on vacant land, or in a yard or area appurtenant
to any building.
Please read and follow these instructions carefully. A correctly completed application will expedite the issuance
of your Special Events License.
1.
A complete Miscellaneous application (BRA-4) is required and must be submitted at least ten (10) work-
ing days before the event. When setting the date for your special event, please include adequate time for
resubmission in case your application is deficient and supplemental information required. It is recom-
mended that the application be submitted 45 to 60 days in advance.

Submit a completed Special Events Questionnaire.

If mechanical amusement devices are to be used, liability insurance is required. An insurance certificate
comprising of one hundred thousand dollars ($100,000) for bodily injury to one person and three hundred
thousand ($300,000) for bodily injury from one occurrence is required. Liability insurance may be ob-
tained from any insurance company. Please be advised only an original insurance certificate will be ac-
cepted. D.C. Government is the certificate holder. The certificate holder shall read: D.C. Government,
941 North Capitol Street., N.E. Room #1100, Washington, D.C. 20002.

Submit a letter authorizing use of property for special event. This letter must be signed by the property
owner or manager.

Submit a letter which describes the location, date(s) and time the event will be held and must indicate the
date and time you plan to complete the construction and to set-up the sales booths, equipment, amuse-
ment rides, etc. The determination of your set-up date is crucial to the process of issuing a license. Care-
ful planning is necessary. On that date, your site will be inspected, and must be approved by food service,
plumbing, construction, electrical and fire safety inspectors.

If a street closing is needed, submit copy of application for street closing from the Emergency Manage-
ment Agency, 2000 14 th Street, NW, 8 th Floor. For additional information, please call (202) 727-3159.

Submit a map secured from the Office of Administrative Services. Indicate on the map the location of the
proposed activity, together with all of the area within a distance of five hundred feet (500) from the
perimeter of lot(s), reservation(s), or parcel(s) of ground to be occupied by the activity and the location
of all properties wholly or in part within that area.
8.
Submit a” consent list” of 90% of the resident housekeepers and occupants of business establishments within a distance of
five hundred (500) feet from the perimeter of the lot where the activity will take
