GENERAL INSTRUCTIONS:
1. The State Board of Tax Commissionerswill review a current assessment under this petition pursuant to IC 6.1.1-11-7 or IC 6-1.1-11-5, only if:
a. the petition was filed with the County Assessor within 30 days after County Property Tax Assessment Board of Appeals' notice of
action on exemption is given to the taxpayer;
b. the petitioner has attached a copy of the application for exemption (Form 136) and a copy of the Notice of Action on Exemption Application (Form 120);
c. the petitioner has fully completed SECTION I, SECTION ll, and SECTION Ill of this petition;
d. the petition is signed by the owner or attorney at law.
2. Petitioners wishing to appeal more than one parcel must file a separate form for each parcel. Please attach a list of related parcels currently under appeal.
3. Please print or type.
4. If there is any change in your address or telephone number, please notify the County Assessor and the State Board of Tax Commissioners.
5. As a result of filing this petition, the exemption percentage may increase, may decrease, or may remain the same.
If a petitioner files a timely petition but fails to comply with the instructions for completing this form (FORM 132), the State Board of Tax
Commissioners will return the form to the petitioner and notify the petitioner of any defect in the completion of the form. The petitioner will then have
thirty (30) days from the date of the defect notice to correctly complete the form and resubmit it to the State Board. If the petitioner does not timely
respond to the defect notice or the resubmitted form does not comply with the instructions for completing the form, the State Board will deny the petition.
If the owner does not comply with the statutory procedures for obtaining an exemption, he waives the exemption. If the exemption is waived, the property is
subject to taxation. IC 6-1.1-11-1
