2000/2001 Remittance Worksheet Instructions
I. Filing Requirements and General Instructions
A. Introduction
On April 4, 1996, the State Corporation Commission of the State of Kansas (KCC) issued an
Order to create the Kansas Universal Service Fund (KUSF) to administer the collection and
distribution of universal service support programs. The KUSF supports high cost rural service,
low income customers, and residential customers who could experience rate shock.
K.S.A. 66-2002 established the KUSF, effective January 1, 1997, to preserve and enhance
universal service, protect the public welfare, ensure the continued quality of telecommunications
services, and to open local telecommunications to competition in compliance with the Federal
Telecommunications Act of 1996.
On January 19, 2000, the Kansas Corporation Commission issued Order 5 (Docket No.
00-GIMT-236-GIT) establishing the assessment rate for providers of wireline
telecommunications services in Kansas at 4.92 percent of intrastate retail revenues,
effective March 1, 2000. For wireless carriers, the assessment rate is 4.66 percent of
intrastate retail revenues, effective March 1, 2000.
B. Who Must File
The Kansas State Act KSA 66-2008(b) dictates that the Kansas Corporation Commission Ashall
require every telecommunications carrier, telecommunications public utility and wireless
telecommunications service provider that provides intrastate telecommunications services to
contribute to the KUSF on an equitable and nondiscriminatory basis. Any telecommunications
carrier, telecommunications public utility or wireless telecommunications service provider which
contributes to the KUSF may collect from customers an amount equal to such carrier’s, utility’s or
provider’s contribution.@
C. When and Where to File
The current KUSF contribution period will include intrastate revenues earned between March 1,
2000 and February 28, 2001. Monthly worksheets and remittances must be received by the

day
of each month, with the first submission at the new assessment rate due on April 14, 2000.
