REMINDERS FOR FORM 499R-2/W-2PR
Box 1 - Show first name and middle initial in first section;
show surname(s) in second section. The employee’s name should
be as it appears on his (her’s) Social Security card.
Show
employee’s mailing address in the third section.
Box 3 - Verify correct SSN is reported for each employee.
Use
the correct format XXX-XX-XXXX.
Box 6 - Verify the correct Employer Identification Number is
reported.
Use the correct format XX-XXXXXXX.
Box 7 – In the case of anuities or pensions, show the total
contributions made by the employee to his retirement plan,
prior to retirement. This same amount will be repeated in
subsequent years.
Box 8 – The term salary signifies all remuneration for
services rendered by an employee to his employer and all
remuneration paid by any means that is not money. Show the
total amount of salaries paid during the calendar year.
Exclude cash or deferred contributions, if any, to be shown in
box 16.
Also include pension payments to retired employees
and identify them as pension.
The amount must be shown
without the dollar sign or commas.
Decimal dots should be
noted.
Box 9 – Show the total of commissions paid to the employees
during the calendar year.
Exclude any cash or deferred
contribution to be shown in box 16.
Show the amounts in the
same format as previously explained.
Box 10 – Show the total amount of allowances (not
reimbursements) paid to the employee during the calendar year
for which no expense report was required.
Box 11 – Show the total amount of tips subject to withholding.
Tips constitute remuneration for personal services.
When the
employer has control of the tips, they must be included as
income for the year received, and they will be subject to
withholding, the same as wages.
Box 13 – Show the amount reimbursed to the employee for
expenses covered by the employee, and for which the employee
was required to submit a report and evidence of expense.
Box 14 – Show the total amount of commonwealth income tax
withheld, including any withholding by third pary payers for
sick pay.
Box 18- Show wages up to the yearly maximum taxable for Social
Security. Verify you report the maximum for the year being
reported, as this may change from year to year. As in the
