VOTER REGISTRATION FORM
TEN SIMPLE STEPS TO REGISTER TO VOTE
1. Print your name clearly. Put your last name first, then your first name and middle initial.
2. Enter the name of the county in which you live.
3. Enter a physical address. This is necessary because the election administrator needs to
determine which voting precinct you live in. If you need more space, you may use the back of
your voter registration form.
4. Enter your mailing address. This will ensure that you receive voting information, such as the
official state voter information pamphlet.
5. Enter your telephone number. This allows the election administrator to contact you with election
questions or information.
6. Enter your birthdate in month/day/year sequence.
7. If you have changed your name, enter your former name to ensure that you are not registered to
vote twice. If not applicable, you can leave this space blank or write “NA” for not applicable.
8. Enter the place where you were last registered to vote, using the names of the appropriate city,
county, and state. This is to ensure that you are not registered to vote twice.
9. Read the voter declaration and then sign and date the bottom of the card.
10. Mail or deliver the card to your county election administrator.
