AT THE TOP OF THE PAGE, THE BORROWER NAME (last, first, middle) AND SOCIAL SECURITY
NUMBER MUST BE REPEATED FROM THE FIRST PAGE.

IF COSIGNER INFORMATION IS NOT AVAILABLE, ALL COSIGNER BOXES (#15 - #24) MUST BE

Cosigner Name
Current name of the cosigner of the loan. May not be
(Last, First, Middle)
blank if there is a cosigner. Must begin with a letter of
the alphabet. Must be entered Last, First, Middle (if
any).

Cosigner SSN
Cosigner’s Social Security Number (SSN) as it
appears on the application. If cosigner data is
available but SSN is not, enter zeros as SSN. Must not
be the same SSN as the borrower.

Cosigner Street Address
Last known street address (house/apartment number
and street name) of the cosigner, even if it is known to
be a bad address. If unknown, enter NO STREET
ADDRESS.

Foreign Country
Last known country of cosigner residence if a foreign
country. May be blank.
