TO BE USED IN CONNECTION WITH AN APPLICATION FOR TITLE AND/OR REGISTRATION FOR A TRUCK,
CLASSES 1 THRU 4B ONLY
This form must be completed, signed and attached to Form MV-1, “Application for Certificate of Title,” Form MV-41,
“Application for Correction of Vehicle Record or Verification of Vehicle Identification Number,” or Form MV-4ST, “Vehicle Use
and Tax Return/Application for Registration.” NOTE: When changing the registered gross weight of a vehicle which is
already titled and registered in your name (on Form MV-41) your title need not be attached.
Vehicle description as well as applicants name and signature must agree exactly with information on accompanying
application. Manufacturer’s GVWR must agree with the weight listed on the certificate of origin, Pennsylvania Certificate of
Title and/or VIN tracing/verification on the accompanying application.

I/We request that the above described vehicle be registered at the gross vehicle weight listed below under the provisions of
Section 1916(b) of the Vehicle Code as amended by Act No. 8 (1980). I/We acknowledge that I/we have been warned by
the Department of Transportation that loading my/our truck beyond the manufacturer’s gross weight rating may damage the
truck and endanger its occupants, as well as other vehicles and their occupants and pedestrians; and I/we assume all risks
connected with any such overloading of the truck.

PERTINENT VEHICLE CODE PROVISIONS
Section 1916(b) - OPTIONAL REGISTRATION - Any vehicle falling within the range of weights for Classes 1 thru 4B, inclusive, shall
notwithstanding any gross vehicle weight stamped on the manufacturer’s serial plate, be registered, upon request of the person making
application for registration, at the maximum allowable gross or combination weight for the particular weight class within which the gross
vehicle weight determined by the manufacturer causes such vehicle to fall.
EXAMPLE: A vehicle with the VIN plate stamped 8,700 lbs. as the GVWR (gross vehicle weight rating) falls into the Class 3 category.
However, this vehicle may be registered at the maximum allowable weight for this class at 9,000 lbs.
WARNING
The operation of a truck loaded beyond the manufacturer’s Gross Vehicle Weight Rating (GVWR) may create unsafe conditions
and may also void the manufacturer’s warranty if damage should result from such overloading. Check with your dealer or factory
representative. You should also consult your insurer concerning possible adverse effects to your insurance coverage with respect
to such overloading.
