The completed application, supporting documentation and proper fee must be mailed via the United
States Postal Service to the address below. Please be advised that Federal Express and UPS will not deliver
to PO Boxes.

Print a completed copy of the application for submission to the Authority.
Save a completed copy of the application to retain for your records.
This application is to be used by a currently licensed manufacturer seeking to operate a Custom
Beermakers’, Cidermakers’ or Winemakers’ Center, provided the applied for premises is currently
licensed for the manufacture of the applied for type. Manufacturers who hold one or more eligible license
types at the same location may apply for multiple Custom Manufacturing Center licenses. If more than
one type of Custom Manufacturing Center license is being applied for, a fee for each license is required.

*If the applicant plans to alter their premises in order to add a Custom Manufacturing Center, an
alteration application is also required, unless the location is a Farm Winery or Micro Winery.
Description of a Custom Manufacturing Center
A Custom Manufacturing Center authorizes the license holder to operate a facility for individuals to
rent space, equipment and storage for the manufacture of alcohol for personal household
consumption. The license holder may also sell beer/cider/wine making supplies to patrons of the
center.
