LIMITED PERMISSION TO REPRODUCE SCENTSY INTELLECTUAL PROPERTY
Any product or service bearing any Scentsy mark, logo, words, phrases, symbols, designs or photography
(“Scentsy’s Intellectual Property”) must be approved by Scentsy, Inc. in writing prior to its production.
Most products require a written licensing agreement between Scentsy, Inc. and the party producing
items which bear Scentsy’s Intellectual Property. In certain circumstances, Scentsy, Inc. may allow
limited production of items bearing Scentsy Intellectual Property.
This form must be completed and returned to Scentsy, Inc., and the production authorized by Scentsy,
Inc. before the production of any item bearing Scentsy’s Intellectual Property.

I understand that Scentsy®, Scentsy Independent Consultant™, images, colors, and designs which are
being reproduced on the item described above, a sample of which is attached to this sheet, are the
intellectual property of Scentsy, Inc. and that only a duly authorized representative of Scentsy, Inc. can
grant permission for me to reproduce the same. This form is not valid until signed by an authorized
Scentsy representative and only grants me a limited license to produce the item described in this form,
and then only in the quantities stated on the form. Any variation in design or quantity will be deemed a
violation of this agreement and of Scentsy, Inc.’s intellectual property rights. Any violation of this
agreement will be subject to the maximum penalties allowed by law. This agreement is subject to, and
interpreted in accordance with the laws of Idaho, and by signing this agreement I agree to be subject to
the jurisdiction of Idaho courts. I also agree that in the event any dispute concerning my performance of
this agreement, violation of this agreement, or production of other material which violates Scentsy’s
Intellectual Property Rights, jurisdiction and venue for that dispute will be in Ada County, Idaho.
