The location where you received this form will remain confidential and will be used only for registration
administration purposes.
If you decline to register to vote, the fact that you so declined will remain confidential.
You must register with a party to vote in that party's primary unless that party allows unaffiliated voters to vote in its
primary. In Section 7, if you indicate a political party that is not qualified or indicate no choice you will be registered
as "Unaffiliated."
