I certify that the students whose names appear below are eligible to represent the TSSAA _________________ High School of

__________________, TN, in the TSSAA ______________ Tennis Tournament to be held at ________________ under the direction

of _____________________.

INSTRUCTIONS: List dual meet won-lost record of each player at each position. This form must be completed and in the hands of the

tournament director and the state office at least five days before the event is scheduled to begin. Players must be listed in order of

ability. Doubles teams will be listed in order of ability. Singles players and doubles teams will always play in order of ability.

Singles substitutions will be made from the bottom up. Doubles substitutions must be a straight-line substitution if for one player. If

substitution(s) are made in doubles, then the order of the doubles teams should be set or changed based on ability.

If any combination are out for any reason, then individual substitutions must come from your singles list and the doubles teams

can be set in any order, and then placed by team in #1 and #2 in their order of ability.

Tournament play in Division I in the region, sectional and state will be 5 singles and 2 doubles. Tournament play in Division II

Class A in the sectional and state will be 5 singles and 2 doubles. Tournament play in Division II Class AA in the first round,

quarterfinals, and state will be 6 singles and 3 doubles.

coach or sponsor and that he or she will accompany the contestants to the tournament, and will have official charge of their activities.

The tournament director is instructed to admit no team until this form has been filed with them and the state office. If any

contestant on the sheet qualifies for the next higher tournament, the tournament director shall mark off the names of the contestants

that fail to qualify, and forward this form to the regional or state tournament director.
