4MEMORANDUM OF AGREEMENT
BETWEEN

(the “midwife”)
AND
Y
(the “other party”)
Preamble
WHEREAS THE PARTIES WISH TO COLLABORATE on a project described below and whereas
XX (the “midwife”) has certain regulatory obligations that she must meet, the parties to this
agreement hereby agree to the following:
Description of Project
1. The purpose of the project is to [describe scope of project here].
2. The project will require the midwife to work in Alternate Practice Arrangements, i.e.,
arrangements that are different than the arrangements by which most midwives in
Ontario work.
3. The project may involve the midwife practising outside of the scope of practice of
midwifery as defined in the Midwifery Act, 1991. If the midwife works outside of the
scope of practice of midwifery then she will be doing so only pursuant to the auspices of
the project and in collaboration with the other party, with whom the authority rests, to
this agreement.
