List the names and complete mailing addresses of the ward/incapacitated person/protected
person and all living spouse, parents, children, two closest known relatives to the
ward/incapacitated person/protected person and all other interested parties as defined below.
Failure to list such relatives and interested parties and notify them of any and all hearings may
result in an order postponing the hearing. The court must be notified within three days of a change
of residence by any person listed on this document. The guardian/conservator must receive
court permission prior to moving the ward/incapacitated person/protected person out of the
State of Nebraska.
