Dear Parents,

Please read over and sign this consent form. By checking each appropriate box, you are

acknowledging or declining consent for each specific item. This consent form is valid for one year –

September 1, 2017 to August 31, 2018.

1. I am allowing my son/daughter to meet one-on-one with their youth director/youth

leader upon request of the youth at a public place. Note: At no time, will the youth

director/leader of the opposite sex meet with a student one on one. Two leaders will

be required to meet at the same public place when it involves the opposite sex.

2. The youth director/leaders can contact my son/daughter to meet in a public place.

Note: At no time, will the youth director/leader of the opposite sex meet individually

or one on one with a student. Two leaders will be required to meet at the same

public place when it involves the opposite sex.

3. I agree when the youth director/leader meet with my son/daughter in a public place

both will arrive and leave by their own means of transportation.

4. I want to be notified when the youth director/leaders meet with my son/daughter.

5. My son/daughter can text the youth director/leader to communicate with each

other.

6. The youth director/leader can text my son/daughter to communicate with each

other.

7. I am stating that I will pick up my son/daughter on time after any and all youth

events. If I am not able to pick up my youth personally, I will arrange other

transportation for my child and will notify the youth leader of these changes.

8. I am stating that my son/daughter will wear his/her seatbelt in any and all vehicles

on all youth events. If this does not occur, the driver will pull vehicle over and stop

the vehicle until the youth complies.

9. I give permission to the adult leaders to search my son/daughter’s possessions if

there is reasonable suspicion of questionable items present. In the event such items

are found, they will be confiscated and the parents will be notified immediately.

10. I want to receive copy of the general text messages and emails that are sent out to

notify my son/daughter of activities, events, meetings and any other general

messages.
