INSTRUCTIONS
This form is used to make the following corrections to an Oregon title or salvage title:
(1) ehicle year, make, body style, or identification number;
(2) orrect the name of the registered owner(s) or security interest holder(s);
(3) emove a brand such as “previous damage” or “totaled” placed on the title in error (We must
also receive a statement from the person or firm who notified DMV or evidence of DMV error )
(4) dometer reading; (Need odometer disclosure and evidence to support the original disclosure

assembled or reconstructed vehicle).
This form c c annot be used to change a name on itle ocuments due to marriage or divorce, or to s s how a new address. You
must submit Form # 735-226 - Application for Title and Registration, along with applicable fees.
There is no fee to correct a vehicle identification number or add or remove “survivorship” to an Oregon title or salvage title
which is in joint ownership.
If this is the form you need to use, complete it in full and sign where indicated. If the vehicle identification number needs to
be corrected, you may be requested to obtain an inspection of this number.
If the title is held by a security interest holder, we suggest you ask for their assistance since the title or salvage title must be
submitted in order to be corrected. A new corrected title will be returned to the security interest holder.
You may take this form with the Oregon title or salvage title and the title fee, if applicable, to your local DMV office or mai l it
