Risk Assessment Form Instructions
Identify the possible hazard for the proposed off-site activity and then using rating chart below, determine
the level of risk (high, medium, or low) for each hazard. For example, a hazard with exposure of 3,
probability of 0.6 and a consequence of 10 would give a risk score of 18 (3 x 0.6 x 10) or a HIGH risk
score. Once all hazards have been evaluated, a decision can be made upon whether or not to proceed.
(NOTE – please refer to the list of prohibited and high risk activities before completing this form for
those activities)
