and wish to establish custody.
Information you will need:
1 The exact name and address of the other party in this
matter.
2. The exact name(s) and date(s) of birth of all children
included in the Petition for Custody.

custody order has already been issued by the court, you may want to
petition the court to modify custody rather than establish custody. The law
allows the court to set all custody orders according to the best interest of
the child(ren).
