The following instructions were specially written to help you, as a first-time filer,

properly complete your first "Employer's Contribution and Payroll Report".

Realizing that filling out a new tax form can be a frustrating experience, we have

tried to answer some of the more commonly asked questions about the unemploy-

ment tax law as well as giving you a step-by step guide for filling out this form.

Please, read these instructions completely before filling out your first report. If we

fail to answer all of your questions, please contact the field auditor at the nearest

Iowa Workforce Center. We have provided a listing of our field office locations and

telephone numbers with these instructions.
