IEP AT A GLANCE
The IEP at a Glance is a read-only form that will summarize key areas of the IEP. It can be given to teachers,
administrators, and parents. This form will replace the current IEP Information sheet. The form will include the
following:
Student Information
Student Name
Grade
•
•
•
Determinations
•
Student ID Number
•
Building
•
Secondary Disability
•
DOB
•
Case Manager
•
Tertiary Disability
•
Age
•
Primary Disability
IEP / Special Education Form based Information
Accommodations (There are two
•
BIP
•
columns - Accommodations Area, and
•
Special Transportation
Accommodation. The accommodation
•
Assistive Technology
area will list the type of assessment (if
•
Student Strengths (Information for this
applicable), and the Accommodation
field is pulled from Present Levels of
column will list the accommodation
Academic Achievement and Functional

Creating an IEP at a Glance (After 1/15/16 all new IEP packages will automatically have the IEP at at
Glance included in the package).
To create an IEP at a Glance form in a finalized package, unfinalize the current IEP package, click

on the + to create a new form.
•
Search for IEP at a Glance and click on Create an IEP at a Glance. Click on ‘Go’.
When the form is created click on the paper icon on the far right, review the document. If there
•
is incorrect information on the form it will need to be corrected through the IEP. This form is
read only. After reviewing the document lock it and refinalize the package.
