CHECKLIST FOR A VISA APPLICATION FOR TOURISM

Duly filled out and signed Schengen visa application
form.

One recently taken passport-sized photo (not older than
6 months, 35-40mm in width, be taken with a light blue
or grey background , full face, no sunglasses.
Please do not affix the photo on the form.

Current valid passport with at least 2 blank pages/at
least 3 months valid upon return and copies of the
holderpage of the passport and all pages with stamps.
Passport should be issued within the last 10 years.

Previous passports.

Proof of travel:
Copy of the roundtrip airline reservation with
passenger details and cop of the travel itinerary.
The reservation should prove that the stay in
Schengen does not exceed 90 days. It is
recommended not to buy the ticket until the visa
is granted.

Proof of Travel Medical Insurance coverage (EUR 30.000)
and a copy:
The insurance must cover the entire period of
intended stay, valid for all Schengen States and
covering any expenses which may arise in
connection with repatriation for medical reasons,
urgent medical attention, emergency hospital
treatment or death during the stay.

Proof of accommodation:
Hotel confirmation or hotel voucher (to all
Schengen states of destination) and a copy.
Copy of the guarantee&hosting form if you are
staying with your host in the Netherland. (This
document can be obtained by the host from
his/her municipality and should not be older than
6 months) and a copy of the host’s passport
(holderpage) or residence permit.

Proof of financial means of the applicant. One or more of
the following documents should be submitted:
Recent personal bankstatement over the past 3
months and a copy.
Salary statements over the past 3 months
Income tax paper and a copy
