Form Instructions
For each set of plates, there is a $5 mailing fee.
MILITARY VETERANS PLATES - (SDCL 32-5-108, 109, 109.1, 109.2) - *Plates can only be used on an automobile, pickup, or van
licensed under the noncommercial license fee schedule or a motorcycle. Qualifying residents of this state may apply for Purple Heart (PH)
(copy of DD Form 214: discharge papers must be furnished); Disabled American Veteran (DAV); Prisoner of War (POW); or Pearl Harbor
Survivor (PHS) license plates. Applicants are limited to two sets of each of these plates. Upon initial application, the applicant must pay the
registration fee for the current year and the $10 special license plate fee; upon verification of eligibility, the registration fee is refunded. If
an applicant dies or otherwise becomes disqualified, the plates must be surrendered to the county treasurer of the county where the
applicant lives. Application is made to the county treasurer of the applicant’s county of residence. *A veteran may choose to use one of the
two sets of military veteran plates on a noncommercial vehicle that is a pickup truck that weighs more than 6,000 pounds licensed under the
declared gross weight vehicle fee schedule or a motorhome. In this case, the veteran pays the regular license plate fees and does not pay the
$10 fee for the special plates and renewal stickers.
NATIONAL GUARD – (SDCL 32-5-55) – A resident of this state who owns a vehicle and who is a regularly enlisted or commissioned
member of the SD National Guard, other than an inactive member thereof, or a retired member with 20 years or more of creditable service
may obtain National Guard license plates. If discharged, separated, or furloughed to a reserve or inactive status, the plates must be
surrendered. Application is made to the county treasurer of the applicant’s county of residence.
MEDAL RECIPIENT PLATES - (SDCL 32-5-109.6, 32-5-109.7)- A resident of this state who owns a vehicle and possesses a South Dakota
driver's license and who has received the Silver Star, Distinguished Service Cross, Navy Cross, Airforce Cross, Distinguished Flying Cross,
Bronze Star with Valor Device, or Bronze Star Medal may obtain Military Medal license plates. A copy of DD214, a copy of the certificate
awarded upon presentation of the medal, or other official documentation that substantiates eligibility is required. If an applicant dies, plates
may not be displayed beyond the expiration of the plates or renewal stickers, plate fee - upon renewal, the registration fee is assessed but no
special plate fee is charged.
GOLD STAR (FAMILY) - (SDCL 32-5-109.5) - A resident owner of a (non commercial) motor vehicle who is a parent, spouse, sibling, or
child of a member of the US Armed Forces who died in service to this country or who died as a result of the service. Application is made
to the country treasurer of the applicant's county of residence. See front of application for requirements.
