SAMPLE JURY EXCUSE LETTER

Dear Local Commissioner of Jurors:

I am writing to request an excuse from jury duty on the grounds that such jury services would

pose an undue hardship on me and an extreme inconvenience to the patients of my dental

practice. I am a sole practitioner and have no other dentists in my office who can treat my

patients if I am not here. This disruption to the care of my patients is a serious inconvenience to

them and to their dental health. In addition, it is an undue hardship on my livelihood to close my

practice for an extended period of time, since my practice depends on my being there for my

patients.

I hope that you will take into account the unique situation of a sole practitioner dentist like

myself, where jury service could substantially impair my livelihood and the health and welfare of

my patients, and see fit to grant my request for an excuse from jury duty.
