A. Instructions
(i) This form is to be completed by ESMC based on social screening checklist submitted by IA
(ii) The social categorization of a project is a continuing process. If there is a change in the components
or/and site of a project that may result in category change, the IA should submit new/updated social
screening checklist, based on which ESMC will complete a new social categorization form. The old
form should be attached for reference.
