This is a notice to request information regarding your sales activity. Please complete this form

and mail your response to the South Carolina Department of Revenue within 30 days upon

receipt of this notification. If you are required to register to remit the Prepaid Wireless 911 Fee

and fail to do so, you will subject to the appropriate fees, penalties and interest for failing to

file the return.

Please note: If you later become a seller of prepaid wireless telecommunications services (e.g. a

seller of wireless phone cards which load (add) wireless airtime to a mobile phone when it runs low,

or prepaid wireless cellular phone plans which include minutes of airtime, etc.), you must notify the

SC Department of Revenue when you begin offering prepaid wireless telecommunications service

to consumers and that you will need to receive the forms to remit the prepaid wireless 911 charges.
