REQUIREMENTS FOR PETITION OF LIMITED DRIVING PRIVILEGES:
If you have been notified by the Ohio BMV of an impending license suspension and you are
a resident of either the city of Lebanon or Turtlecreek township; OR you have recently
received a citation in the Lebanon Municipal Court and failed to show proof of insurance,
you may file a civil suit to secure limited driving privileges. In order to do so you must fill out
this Petition form and provide:
✓ $80.00 ($70.00 filing fee + $10.00 fee for the document)
✓ Proof of valid insurance
✓ Suspension notification from the BMV showing your BMV case number and the
dates of your suspension*.
✓ Copy of your driving record*
✓ Letter (on company letterhead) from your supervisor detailing days and hours
you normally work
✓ Current school schedule (if applicable)
✓ Ohio driver’s license (or a document that shows license number and expiration
date)
✓ Receipt for any outstanding reinstatement fees owed to the Ohio BMV
* If you no longer have the suspension notification form, you can print out a listing of reinstatement
requirements and a copy of your driving record from the “BMV online services” section of
Once you have obtained all the necessary documentation, fill out the top of the Petition
and bring all paperwork to the Clerk’s office any Monday or Thursday, between the
hours of 2:00pm and 4:00pm and your case will be processed and heard by the judge. If
approved, your letter to drive will be processed for you and you must then register the
letter at the Ohio BMV’s regional reinstatement office in Mt Healthy, Ohio in order to
validate your privileges. Failure to register the letter may result in future citations and/or
arrest for driving under suspension.
