Provide a complete description of the boat for which you are registering and/or obtaining title. Any
missing data could delay application processing.

Enter the information from the capacity plate on your boat. All boats less than 20-feet designed to carry two
or more people are required to have a legible capacity plate (except canoes, kayaks, sailboats, pontoon boats,

personal watercraft and hydroplanes). Capacity plate must be displayed on the boat prior to transfer of
ownership. Failure to provide this information will delay processing. If the boat does not have a capacity
plate, complete Form PFBC-702 available from the PA Fish & Boat Commision.

Outboard Motor Data
Complete this section only if your lien holder requires outboard motor data on the title.
Section F
Lien Information
If there is a lien against the boat, you must complete this section. Title will be sent to the lien holder.
Section G
Seller Information
Name of person or business selling the boat.
Section H
Agent Information
To be completed only by an authorized issuing agent of the Fish and Boat Commission.
Enter complete purchase information and fees for registration and/or titling. Purchase price of boat includes
boat, motor(s), accessories, dealer preparation charges and destination or freight charges. Do not include
price of trailer in purchase price of boat, rather use Pennsylvania Department of Transportation Form MV-1 to
