INSTRUCTIONS FOR FILING MODIFICATION TO COVENANT AND
AGREEMENT FORMS
Note: This Supplemental Covenant & Agreement Form is to be used to modify existing
Covenant & Agreement for previously approved Projects
.
1. Fill out, in BLACK INK ONLY, one copy of the Supplemental Covenant and Agreement (C&A) Form.
2. Property owner(s) must print and sign their name(s) – SIGNATURE(S) MUST BE NOTARIZED.
3. Submit the completed Supplemental Covenant & Agreement (C&A) Form, along with the modified
Operation and Maintenance Plan to the Bureau of Sanitation for approval and signature.
4. Record the Supplemental C&A Form with the Los Angeles County Registrar-Recorder and obtain a
certified copy. County Recorder located at:

5. Return the certified copy of the recorded form to the Bureau of Sanitation requiring the
supplemental covenant (should be a purple stamp on the back of the last page of document
recorded).
NO FINAL APPROVAL/CLEARANCE WILL BE GRANTED WITHOUT BUREAU OF SANITATION RECEIPT OF THE
CERTIFIED COPY OF THE SUPPLEMENTAL C&A WITH THE MODIFIED O&M PLAN.
