Purpose of this Form: This form is to be used by a vehicle owner to request the replacement of a lost, stolen, mutilated or
defective License Plate/Tag or Decal or to report that a License Plate/Tag or Decal was never received. A fee of $8.00 will be
charged for a replacement.* This form should not be used to record a change of ownership, change of address, or change of
license plate classification.

request is submitted within ninety-days of initial tag/decal issuance date in addition to a T-200 and copy of a police report.

Mutilated/Defective: You must submit the tag/decal along with this application.
Never Received in the Mail: Signature and Seal or Stamp of Notary or Justice of the Peace is required on this form.
