This letter is to inform you that we hereby formally end our relationship with you as your [bookkeeping/tax] firm as of [date.] As you

for the year ended (date)]. The current status of your [project/returns] are as follows: [Provide reasonable detail of the status of the
engagement. The greater the detail, the greater the protection to the tax professional.]
Our reasons for disengaging at this time are the following: [Choose the applicable paragraph below]
1. The unpaid fees as last requested in our letter of [date] have still not been paid. Our several communication efforts regarding payment
have not been successful. The outstanding balance in your account with us is $[amount.] In accordance with our agreement, termination
of our relationship does not discharge you from making full payment for services rendered by our firm to date.
2. The incomplete information received to date leaves too many items unresolved to completely and accurately prepare your tax
return(s). We have repeatedly requested the missing information, and again our several communication efforts have been unsuccessful.
In the interests of maintaining high levels of efficiency and service, we have determined that our relationship must be terminated.
3. In accordance with your instructions, our relationship has terminated. We will not render further services to you or on your behalf,
including completion of any unfinished work, unless and until a written agreement to do so has been executed by you and this firm. Your
payment of the fees or providing requested information at this time will not automatically re-establish our relationship. Our relationship
may be re-established only by written agreement between you and this firm. Partial work done by our firm is provided "as is" and may
not be relied upon if there are any changes in facts or circumstances relating to that work.
Please advise this office as soon as possible of your instructions concerning return of any of your materials that remain in our possession.
You may pick the file up at our office (please call for an appointment so that your file will be ready), have us send it to you, or have us
send it to your new tax professional. If we do not hear from you within a reasonable time, we will retain your file in accordance with our
document retention policy. In no event will we be responsible for documents left longer than four years from the date of this letter.
This firm does not issue or release incomplete tax returns. Your returns are on extension until [date]; attached is a copy of the form
[name of specific extension form] [which you signed]. Given the deadline of [date], we urge you to gather your documentation and
engage another tax professional as soon as possible in order to complete your return in a timely manner and to avoid potential
penalties or other adverse consequences. It is your sole responsibility to ensure that appropriate materials are filed on a timely basis.
We will cooperate with your subsequent tax professional if you authorize us to do so. [We are attaching a form for you to complete,
sign and return to this office in the event that you consent to our disclosing tax information to your new tax professional.]
